We request the student’s current schedule to support preliminary service planning and to better understand the student’s current programming needs across the school day. For the purposes of this program, both virtual access and onsite access are considered active school participation and are reviewed together to understand how the student is currently engaging with instruction and supports. All times are listed in your local time zone.
We ask whether the student’s schedule is expected to remain consistent to support intentional planning across program phases. In some cases, participation in the program may involve a planned increase in instructional access, engagement, or independence over time. Understanding anticipated changes helps ensure considerations align with both current and future programming expectations.

The purpose of this screener is to determine whether this student may be an appropriate fit for participation in The Lab. It is not a diagnostic tool and does not constitute acceptance into the program. Please do not include personally identifiable information in your responses. Responses should be written in a way that allows the team to understand the student’s needs without identifying the student directly.
